If your licensure has expired and you would like to reinstate it, please follow the following process for reinstatement of licensure.

 

 

REINSTATEMENT APPLICATION

 

THE REINSTATMENT FEE IS $100

 

To apply for reinsatement you must submit the following to the NCBATE office:

1. Completed REINSTATEMENT APPLICATION and CEUs (see hourly requirements below)

2.  $100 Reinstatement Fee

3. Copy of current BOC card or verification from BOC website of current BOC certification

4.  Protocol


The CEU requirements for reinstatement are as follows:

 

21 NCAC 03 .0303 LAPSED LICENSE

(a) The lapsed license may be renewed within a period of five years after expiration upon payment of the lapsed license fee and the completion of 25 contact hours of continuing education for each year that the license has lapsed.

 

(b) A license that has lapsed for more than five years shall be renewed upon payment of the lapsed license fee and the completion of 28 hours of continuing education for each year that the license has lapsed.
